All visits may include a full nutrition and growth assessment, diet analysis, professional recommendations, meal plans, and educational materials based on the needs determined by your provider.
Please be advised that we require a credit card to hold your appointment reservation. Your card will only be charged in the event that you cancel with less than 24 hours notice, or miss the appointment. Please fill out the form below and our office will contact you to schedule an appointment.
Insurance Information
Out team is experienced and knowledgeable about billing Medical Nutrition Therapy. We process insurance claims on behalf of our patients and families. We make the insurance claim process as simple as possible for families, and the majority of our families do receive partial to full reimbursement for services if nutrition is covered under their health insurance plan.
Please note the type of nutrition coverage you have will depend on your specific plan contracted by your employer. Although this does not guarantee coverage, we are credentialed with the following companies as in-network providers:
- Aetna
- American Specialty Health Network
- Blue Choice
- Blue Cross Blue Shield
- Humana
- MultiPlan/PHCS
- United Healthcare
To determine if nutrition services are covered, you will need to contact your insurance provider and ask if your specific plan includes benefits for Medical Nutrition Therapy, billed under code 97802 and 97803. Benefits may be defined, but not limited to, deductibles, co-pays and co-insurance. Please contact us if you have more questions about your benefits, and our staff will direct you.
We require a credit card to reserve your first appointment. Your card will not be billed at the time of the reservation booking. However, we do require 24 hour notice for appointment changes or cancelations.
At the time of your visit, you will not be charged if all of your paperwork is complete. We will first submit your charges to your insurance company. Dependent upon your coverage, you will receive a bill if there is a balance. All bills paid within 30 days are eligible for a discount.
